What Does High ALT Mean?
High ALT indicates liver cells are damaged or inflamed, leaking the enzyme into blood.
Common Causes
- Fatty liver disease (NAFLD) — most common cause
- Viral hepatitis B or C
- Alcohol use
- Medications — statins, acetaminophen, antibiotics
- Autoimmune hepatitis
- Intense exercise (temporary)
Severity
- Mild (1-3x normal): often NAFLD — repeat in 6-8 weeks
- Moderate (3-10x): investigate — hepatitis serologies
- Marked (10x+): urgent evaluation
Next Steps
- Repeat testing
- Check AST (AST:ALT ratio helps identify cause)
- Viral hepatitis panel
- Liver ultrasound
- Review all medications and supplements
